The great thing about the Old Fashioned is that it's more of a style of drink versus a set recipe, which gives an opportunity to be creative.
The key flavour combination is a spirit base with sugar and a few dashes of bitters finished off with a citrus twist (the cocktail cherry is not used as much these days). The most common spirits used are American and Canadian whiskies.
